| | Registration Information There are four (4) options to register for any course: Steps to Register - Phone Register- Call Hazmat's Training and Consulting Division at (716) 827-7200 ext. 8288 or 1-800-678-2600.
- Fax-Copy and complete the registration form, and fax to (716) 748-8381.
- Mail - Copy and complete the registration form and mail it to Susan Penksa or Gina Herron @ HazMat Environmental Group, 60 Commerce Drive, Buffalo, NY 14218.
Please do not mail in registrations less than ten (10) days prior to the course. *Once your registration is received, a confirmation letter including directions and a hotel list (if applicable) will be faxed or e-mailed within 48 hours.
Cancellations/Substitution Policy:
We require 48-hours notice prior to the course. Late cancellation will be subject to a 25% cancellation fee. Substitutions can be made at anytime.
Payment Options: - Checks made payable to HazMat Environmental Group, Inc.

- We can also Bill pre-qualified companies.

HazMat assumes no responsibility for loss of other expenses. We will notify our customers immediately if a course is cancelled. |
